Payment & Insurance
Please
present your insurance cards, identification card and referral numbers to the
receptionist upon your arrival. Payment is expected at the time of service for
all patient balances. You will be responsible for payment of any deductible
amount, as well as charges not covered or disallowed by your insurance carrier.
In instances of work-related injuries, it is your responsibility to verify that
you have authorized workers' compensation. It is your responsibility to contact
your insurance company or your primary care physician for referrals. Please

Prescription Refills
You may call us during regular office hours to request
medication refills. Please allow 48 hours for refills to be made. If picking up
from our office, please pick up by 3pm on specified day. Please check with your
pharmacy to verify when your prescription is ready. No prescriptions will be
filled after hours or on weekends.
Medical Records
Your medical record is private and confidential and we
protect it under all HIPAA laws. We will routinely forward your medical
information to your referring physician. In instances of work-related injuries,
records of your visit to Pinnacle will be forwarded to your workers'
compensation insurance carrier. If you would like your medical records sent to
anyone else, please be sure that you have signed an authorization form.
There is a $25 charge for all Disability and FMLA forms.
